We hope you are enjoying this time between Thanksgiving and Winter/Christmas break. Before we head out for two weeks off, we are excited to let you know we are in the planning stage of this year’s Silver Mountain Ski/Snowboard Field Trip for our students. I had previously misunderstood the information I was given. I apologize for the confusion. We will be going three different days, leaving school right after attendance and returning in time to catch the bus. Attached you will find all the forms that need to be carefully filled out and signed. Please note, these forms cover all three days, so you will only be filling them out once.
COST:
$15 per day, which includes Gondola pass, equipment rentals, and a lesson - everyone takes a lesson each day. ($45 total)
If the student has a pass and equipment, the cost is $10 per day ($30 total).
Checks can be made out to KMS. We also accept cash.
Money needs to be turned in by January 7.
All students are required to wear a helmet. If the student has their own equipment, but not a helmet, they can get one at the rental shop for no cost.
FORMS: There are two different forms attached. Please fill out both. All forms must be turned in by January 10, (preferably before Christmas break). PLEASE MAKE SURE ALL FORMS ARE SIGNED (check both sides)
Field Trip Permission - this form is required by the school for us to take your students on this field trip.

Please fill out renter information - Name, address, parent email, student birthdate, age, and emergency contact information.
Everyone is taking lessons, so please fill out that section.
Rental information : Please fill it out, even if they are not renting. That way, if something happens and they need to rent, they will be set up and we won’t have to send forms home again. Where it says “Renting,” write NO beside it and then fill it out anyway. Make sure you put shoe size, height and weight.
PLEASE FILL OUT THE SKIER TYPE ACCURATELY. We know the students want to be with their friends, but if they are skiing out of their ability level, it is a long day for everyone. Safety should always be first!
CLOTHING: Students need to have a coat, snow pants, warm gloves (please, no mittens) and two pairs of socks. They may want a hat to wear on the walk over and gondola, but they will be wearing a helmet, so a hat is not necessary.
FOOD: Students will need a sack lunch. They can either bring their own or we will have lunches made by the cafeteria. If they would like to bring money for a snack or hot chocolate/drinks, they may do so. However, they will not be allowed to buy lunch up there, as it will take too long to get everyone through.
BEHAVIOR: Our handbook states “any student with multiple referrals for the school year may not be allowed to attend end-of-year field trips.” As this is not an end-of-year trip, any student who gets an office referral between now and Jan 8 will not be allowed to attend the first ski day. We believe in second chances, so as long as there is not another referral, they will be allowed to attend the remaining trips. However, if they do get another referral, they will stay back for the other two days.
We want the students to have a great time up on the mountain, however, our NUMBER ONE CONCERN IS SAFETY. After their lesson, they will be skiing/snowboarding on their own. They must stay in groups of three, which is why the Skier Type is so important. They are representing Kellogg Middle School, therefore we expect excellent behavior. While on the trip, if students misbehave in a manner that causes danger to themselves or others, are disrespectful to people or property of Silver Mountain, or anything else that breaks school rules, they will not be allowed to attend the remaining days.

EQUIPMENT RENTAL AGREEMENT In consideration of S&W Ops LLC, doing business as Silver Mountain Resort renting the equipment to me pursuant to my application above (“the Equipment”), I hereby agree as follows: 1. I have inspected the Equipment and accept it in its current condition. 2. I accept full responsibility for the care of the Equipment during the term of its rental and agree to return it on the above expiration date in as good a condition as I received it, less normal wear and tear. I agree to pay the cost of any damage to the Equipment caused by my misuse or other casualty to the Equipment during the rental. 3. If I fail to return the Equipment or return it in a damaged condition which is beyond repair, I agree to pay the retail replacement cost of the Equipment. I assume full responsibility for the Equipment being stolen. 4. If the Equipment is not returned by 4:00 pm on the expiration date, I will pay its daily rental until it is returned. 5. I authorize the charging of my credit card for daily rental charges for late return of the Equipment, for costs of repair of the Equipment and for the retail replacement value of the Equipment if it is damaged beyond repair or not returned within one week of the expiration date. 6. I understand that I will be using the Equipment in a recreational activity that is hazardous and could result in my injury or death and assume all risk of injury or death resulting from my use of the Equipment. 7. I agree to properly use the Equipment and assume responsibility for knowing or learning how to use it properly. 8. With respect to renting of ski or snowboard Equipment, I agree that the following shall apply, as the case may be: Silver Mountain Group Rental/ Lesson / Release Form a) I UNDERSTAND THAT THE SKI/BOOT/BINDING SYSTEM WILL NOT RELEASE AT ALL TIMES OR UNDER ALL CIRCUMSTANCES. NOR IS IT POSSIBLE TO PREDICT EVERY SITUATION IN WHICH IT WILL RELEASE, AND IT IS THEREFORE NOT A GUARANTEE OF MY SAFETY. I FURTHER UNDERSTAND THAT THE SKI BINDING IS NOT DESIGNED OR CAPABLE OF PROTECTING AGAINST KNEE INJURIES OR INJURIES TO ANY OTHER PART OF MY BODY. I HAVE VERIFIED THAT THE VISUAL INDICATORS ON THE BINDINGS SHOW A SETTING WHICH MATCHES THE SETTING SET FORTH ABOVE. b) I UNDERSTAND THAT SNOWBOARDING/BOOT/BINDING, TELEMARK SKI/BOOT/BINDING SYSTEM, SNOWBLADE/BINDING AND SNOWSHOE/BINDING ARE NOT DESIGNED TO RELEASE AS A RESULT OF FORCES GENERATED DURING OPERATION, INCLUDING FALLS. c) ACCORDINGLY, I EXPRESSLY AND VOLUNTARILY ASSUME THE RISK OF AND THE LEGAL RESPONSIBILITY FOR ANY INJURY TO PERSON OR PROPERTY WHICH MAY RESULT FROM THE USE OF SKI/BOOT/BINDING SYSTEM, TELEMARK SKI/BOOT/BINDING SYSTEM, SNOWSHOE/BOOT/BINDING SYSTEM. RELEASE OF LIABILITY In consideration of S & W Ops LLC, doing business as Silver Mountain Resort, renting the Equipment to me and/or providing the lessons referenced above, I hereby release S & W Ops LLC and its lessors and their respective employees, managers, members, agents, contractors and affiliates (the “Released Parties”) from any and all claims for my injury or death or damage to my property arising out of my use of the Equipment or my participation in lessons referred above. This release of claims against the Released Parties includes, but is not limited to, claims arising out of failure of Equipment. As further condition for my use of the Equipment and/or participation in lessons, I agree to indemnify and defend the Released Parties against any claims of third parties for injury, death or property damage arising out of collisions with me or any of my actions while I am using the Equipment or participating in Lessons. If any provision of this agreement is found to be unenforceable, it shall be severed, and the balance of the agreement shall remain in full force and effect. Venue for any action related to this agreement shall be in Shoshone County, Idaho or, if federal court jurisdiction is involved, in the federal district court serving Shoshone County, Idaho.
